Key Responsibilities

• Evaluate patients and develop individualized physical therapy treatment plans
• Deliver skilled physical therapy services to residents in a skilled nursing facility setting
• Document patient progress, treatment outcomes, and clinical notes in accordance with facility and regulatory standards
• Collaborate with interdisciplinary care teams to support patient goals and discharge planning
• Educate patients, families, and caregivers on therapeutic exercises and home programs

Required Qualifications

• Active Physical Therapist (PT) license in the state of Ohio
• Graduate of an accredited Physical Therapy program
• Current CPR/BLS certification
• Experience working in a clinical or rehabilitation setting

Preferred Experience / Skills

• Prior experience in a skilled nursing facility or long-term care environment
• Familiarity with Medicare documentation and compliance requirements
• Strong communication and patient-centered care skills
