Key Responsibilities- Evaluate and treat patients in a skilled nursing setting
- Develop and implement individualized physical therapy treatment plans
- Monitor and document patient progress toward therapy goals
- Collaborate with interdisciplinary care team to ensure quality patient outcomes
Required Qualifications- Active Physical Therapist (PT) license in the state of New Hampshire
- Valid state or federal issued photo identification
- Ability to comply with facility badging and identification requirements
- Minimum 36-hour weekly availability guarantee
Preferred Experience / Skills- Prior experience in a skilled nursing or long-term care environment
- Strong documentation and communication skills
