Key Responsibilities

• Provide direct physical therapy services to residents in a skilled nursing facility setting
• Evaluate, develop, and implement individualized treatment plans to support patient recovery and functional goals
• Monitor and document patient progress in accordance with facility and regulatory standards
• Maintain valid state-issued Physical Therapist license throughout the duration of the assignment
• Adhere to facility policies, including wearing a photo ID badge at all times while on site

Required Qualifications

• Active Physical Therapist (PT) license in the state of New Hampshire
• Valid state or federal issued photo identification required for facility check-in
• Ability to commit to a 14-week contract assignment

Preferred Experience / Skills

• Prior experience in a skilled nursing facility or long-term care environment
• Familiarity with Medicare documentation and compliance requirements
