Conduct comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
Design functional treatment programs focused on progressing patients toward discharge within an established length of stay and facilitating effective return to community activities
Delegate and direct discipline-specific therapy services for assigned patients while ensuring care is provided under valid physician orders and within insurance approvals
Serve as an educator for patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations
Adhere to discipline-specific State Practice Act and all regulatory requirements; actively pursue professional development through continuing education and professional organization involvement
Required Qualifications
Active and unrestricted Physical Therapist license in good standing in the state of Colorado
Completion of all state-mandated continuing education requirements
Ability to perform comprehensive patient evaluations and develop evidence-based treatment plans
Proficiency in clinical documentation including progress notes, goal writing, and letters of medical necessity
Commitment to patient and employee safety standards and safe patient handling practices
Preferred Experience / Skills
Prior experience in an outpatient physical therapy setting
Experience supervising or mentoring physical therapy students
Familiarity with performance improvement methodologies and quality initiative participation
