    Job Title

    Physical Therapist – Certified Lymphedema Therapist (Outpatient)

    Contract | Days | 8 Hours/Day | 6-Month Duration | Onsite

    Key Responsibilities
    • Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in alignment with patient needs, goals, and physician direction
    • Conduct 60-minute evaluation and treatment sessions in an outpatient setting
    • Complete accurate and timely medical record documentation including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
    • Design functional treatment programs that support effective patient return to community activities, progressing patients toward discharge within an established length of stay
    • Delegate and direct discipline-specific therapy services for assigned patients
    • Participate in student programs as assigned, providing supervision, developing learning objectives, and completing student evaluations
    Required Qualifications
    • Active Physical Therapist licensure in good standing in the state of Ohio
    • Certified Lymphedema Therapist (CLT) certification required
    • Experience as a licensed Physical Therapist — new graduates will not be considered
    • Adherence to discipline-specific State Practice Act and all applicable regulatory requirements
    • Ability to maintain compliance with all state-mandated continuing education requirements
    Preferred Experience / Skills
    • Prior experience in an outpatient therapy setting
    • Demonstrated commitment to professional development through continuing education and involvement in professional organizations
    • Familiarity with performance improvement methodologies and quality of care initiatives
