Key Responsibilities

• Conduct skilled home health visits, with an expected 6–8 visits per day across patient populations including cardiopulmonary, geriatrics, neuro, ortho, and oncology
• Perform patient assessments encompassing cardiopulmonary/circulatory, neurological, and orthopedic evaluations
• Develop and instruct patients and families on Home Exercise Programs and safety/injury prevention strategies
• Utilize and educate on equipment and devices including crutches, gait belts, rolling walkers, standard walkers, sliding boards, soft/hard collars, shoulder slings, hospital beds, and supplemental oxygen delivery systems
• Complete OASIS documentation and maintain accurate records using designated scheduling and timesheet systems
• Recognize and report signs and symptoms of physical, sexual, and psychological abuse in accordance with applicable protocols

Required Qualifications

• Active Florida or Compact Physical Therapist license required at time of submission
• BLS certification required
• Minimum 2 years of physical therapy experience required
• Home health experience required
• Valid driver's license and personal vehicle required to perform home visits
• Must be COVID-19 vaccinated or have an exemption form on file; flu shot or declination required (mandatory masking applies in either case)
• Locals are welcome — candidates residing within 100 miles of the facility may be eligible at the same rate

Preferred Experience / Skills

• OASIS training preferred
• Cerner EMR experience preferred
• Experience working with geriatric, oncology, or cardiopulmonary patient populations
