    Contract | Days | Full-Time | 5x8-Hour Shifts (8:00–17:00) | Onsite / Home Health

    Key Responsibilities
    • Conduct skilled home health visits, completing a minimum of 6–8 patient visits per day across cardiopulmonary, geriatric, neurological, orthopedic, and oncology populations
    • Perform comprehensive patient assessments including cardiopulmonary, circulatory, neurological, and orthopedic evaluations
    • Develop and instruct patients and families on individualized home exercise programs and safe use of adaptive equipment such as walkers, crutches, sliding boards, and soft/hard collars
    • Complete accurate and timely clinical documentation using facility-designated systems and portable technology provided in the field
    • Educate patients and families on medications, signs and symptoms of abuse, safety, and injury prevention
    • Participate in every 3rd weekend rotation; self-schedule visits and hours based on home care office needs
    Required Qualifications
    • Active Florida state Physical Therapist license or valid compact license at time of submittal
    • Current BLS certification
    • Minimum of 2 years of physical therapy experience, with home health experience required
    • Proficiency in patient assessment, documentation, and use of adaptive equipment and devices
    • Valid driver's license and reliable personal vehicle for conducting home visits
    • COVID-19 vaccination or approved exemption on file; flu shot or declination with mandatory mask compliance
    Preferred Experience / Skills
    • OASIS training or prior OASIS documentation experience
    • Experience with Cerner electronic health records
    • Background working with cardiopulmonary, geriatric, neurological, orthopedic, or oncology patient populations
