Job Title
Physical Therapist (PT) – Long Term Care (LTC)

Key Responsibilities
- Evaluate, diagnose, and treat individuals with movement dysfunctions, pain, and functional limitations due to injury, illness, or disability
- Develop and implement personalized treatment plans utilizing therapeutic exercises, manual therapy, and other evidence-based techniques
- Work to restore patient function, manage pain, and improve overall quality of life
- Collaborate with patients and their families to establish meaningful, goal-oriented care plans
- Educate patients on injury prevention, wellness strategies, and self-management techniques
Required Qualifications
- Minimum of 2 years of physical therapy experience (required)
- Active Texas state physical therapy license or a valid compact license recognized in Texas (required)
Preferred Experience / Skills
- Experience working in a long-term care (LTC) or skilled nursing facility setting
- Demonstrated ability to deliver rehabilitative care in a patient-centered, goal-focused environment
