Conduct comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
Design functional treatment programs that support effective return to community activities and progress patients toward discharge within an established length of stay
Delegate and direct discipline-specific therapy services for assigned patients
Serve as an educator for patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations
Adhere to discipline-specific State Practice Act and all regulatory agency requirements, maintaining professional licensure in good standing and completing all state-mandated continuing education requirements
Required Qualifications
Active and unrestricted Physical Therapist license in the state of Colorado
Ability to perform 60-minute evaluations and 30-minute follow-up appointments in an outpatient setting
Proficiency in clinical reasoning, treatment planning, and functional goal writing
Knowledge of and adherence to discipline-specific professional codes of conduct and ethical standards
Commitment to patient and employee safety practices including safe patient handling and infection control protocols
Preferred Experience / Skills
Experience in an outpatient physical therapy setting
Familiarity with performance improvement methodologies and quality of care initiatives
Experience supervising or mentoring physical therapy students
