Key Responsibilities

• Evaluate and treat patients in an outpatient physical therapy setting
• Develop and implement individualized treatment plans based on patient needs and clinical findings
• Document patient progress and maintain accurate clinical records in accordance with facility requirements
• Collaborate with interdisciplinary team members to ensure continuity and quality of patient care
• Educate patients and families on therapeutic exercises, injury prevention, and recovery strategies

Required Qualifications

• Active Physical Therapist license required; permanent Arizona state license required (compact license not accepted)
• Prior experience in an outpatient physical therapy setting
• Ability to complete facility-specific credentialing and documentation requirements prior to assignment start
• Candidates who have worked permanently or PRN at this facility within the last year are not eligible for this assignment

Preferred Experience / Skills

• Previous travel therapy experience in an outpatient rehabilitation environment
• Strong documentation skills and ability to adapt to facility-specific electronic health record systems
