Screen, evaluate, and rehabilitate patients with physical functioning disorders and/or wound care needs, developing appropriate plans of care in accordance with regulatory and clinical practice standards.
Administer rehabilitation procedures including manual techniques, therapeutic exercises, ambulation, modalities, use of supportive and assistive devices, and wound care.
Assess and document patient progress, adjust treatment plans accordingly, and evaluate treatment outcomes for effectiveness in the electronic documentation system.
Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders and discharge planning.
Communicate effectively with supervisors and the broader health care team regarding patient progress, barriers, and treatment plans.
Supervise and support physical therapist assistants, rehabilitation aides, and/or students as needed, and participate in required meetings, trainings, and in-services.
Required Qualifications
Active Massachusetts Physical Therapist state license required.
Minimum of 1 year of physical therapy experience, or new graduate with completed Level II clinical experience.
Experience in long-term care (LTC) or rehabilitation settings required.
Adherence to APTA standards, state practice acts, code of conduct, and professional ethics.
Preferred Experience / Skills
Familiarity with standardized assessments and evidence-based clinical interventions.
Experience with electronic documentation systems and accurate recording of treatment minutes per regulatory requirements.
