Schedule / Job TypeContract | Full-Time | Days | Monday–Friday, 8:00 AM–5:00 PM | 13-Week Assignment | Onsite / Field-Based
Key Responsibilities- Provide skilled physical therapy services to patients in a home health setting within San Bernardino County
- Conduct thorough patient evaluations and develop individualized treatment plans
- Complete OASIS assessments accurately and in accordance with regulatory requirements
- Document patient progress and clinical notes using the HCHB electronic medical records system
- Meet productivity expectations of 30 points per 40-hour work week
- Collaborate with the interdisciplinary care team to ensure coordinated, patient-centered care
Required Qualifications- Active and unrestricted Physical Therapist (PT) license in the state of California
- Minimum of 1 year of home health experience
- Minimum of 1 year of OASIS experience
- Proficiency with HCHB (Homecare Homebase) EMR system
- Valid driver's license and reliable transportation for patient home visits within San Bernardino County
Preferred Experience / Skills- Experience managing a high-volume caseload in a home health environment
- Strong documentation and time-management skills in a field-based setting
