Key Responsibilities
Complete comprehensive evaluations of assigned outpatient patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
Conduct 60-minute initial evaluations and 30-minute follow-up appointments, ensuring progression toward discharge within an established length of stay
Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
Delegate and direct all discipline-specific therapy services for assigned patients, ensuring care is provided under signed, valid physician orders and within insurance approvals
Serve as an educator for patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations as assigned
Adhere to discipline-specific State Practice Act and regulatory requirements, maintaining professional licensure in good standing and completing all state-mandated continuing education requirements
Required Qualifications
Active and unrestricted Physical Therapist license in the state of Colorado
Ability to perform comprehensive patient evaluations and develop evidence-based treatment plans
Proficiency in medical record documentation in compliance with state, federal, and payer guidelines
Knowledge of and adherence to discipline-specific professional code of conduct and State Practice Act
Preferred Experience / Skills
Prior experience in an outpatient physical therapy setting
Experience supervising or mentoring physical therapy students
Familiarity with performance improvement methodologies and organizational quality initiatives
