    Key Responsibilities
    • Conduct comprehensive patient evaluations (60-minute evaluations and 30-minute follow-up visits) and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
    • Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
    • Design functional treatment programs that support effective patient return to community activities, ensuring care is provided under valid physician orders and within insurance approvals
    • Delegate and direct discipline-specific therapy services for assigned patients while upholding professional standards and applicable codes of conduct
    • Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations
    • Actively pursue professional development through continuing education, self-study, and involvement in professional organizations, applying learned information to meet organizational objectives
    Required Qualifications
    • Active and unrestricted Physical Therapist license in good standing in the state of Colorado
    • Adherence to the state-specific Physical Therapy Practice Act and all applicable regulatory agency requirements
    • Completion of all state-mandated continuing education requirements
    • Demonstrated ability to perform comprehensive patient evaluations and develop evidence-based treatment plans
    • Commitment to patient and employee safety standards including safe patient handling, infection control, and workplace hazard awareness
    Preferred Experience / Skills
    • Prior experience in an outpatient physical therapy setting
    • Experience supervising or mentoring physical therapy students
    • Familiarity with performance improvement methodologies and quality of care initiatives
