Conduct comprehensive patient evaluations (60-minute evaluations and 30-minute follow-up visits) and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
Design functional treatment programs that support effective patient return to community activities, ensuring care is provided under valid physician orders and within insurance approvals
Delegate and direct discipline-specific therapy services for assigned patients while upholding professional standards and applicable codes of conduct
Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations
Actively pursue professional development through continuing education, self-study, and involvement in professional organizations, applying learned information to meet organizational objectives
Required Qualifications
Active and unrestricted Physical Therapist license in good standing in the state of Colorado
Adherence to the state-specific Physical Therapy Practice Act and all applicable regulatory agency requirements
Completion of all state-mandated continuing education requirements
Demonstrated ability to perform comprehensive patient evaluations and develop evidence-based treatment plans
Commitment to patient and employee safety standards including safe patient handling, infection control, and workplace hazard awareness
Preferred Experience / Skills
Prior experience in an outpatient physical therapy setting
Experience supervising or mentoring physical therapy students
Familiarity with performance improvement methodologies and quality of care initiatives
