Contract | Days | Monday through Friday with 2 evenings until 6:00 PM | Outpatient
Key Responsibilities
Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
Design functional treatment programs to support effective return to community activities and progress patients toward discharge within an established length of stay
Provide supervision, guidance, and direction of discipline-specific therapy services for assigned patients, including delegation as appropriate
Participate in student programs as assigned, providing a positive learning environment with appropriate supervision and completion of student evaluations
Adhere to discipline-specific State Practice Act and regulatory requirements, maintaining professional licensure in good standing and completing all state-mandated continuing education requirements
Required Qualifications
Active and valid Physical Therapist license in the state of Georgia (or eligibility to obtain)

Ability to perform comprehensive patient evaluations and develop evidence-based treatment plans
Knowledge of and adherence to discipline-specific professional codes of conduct and state practice acts
Ability to maintain accurate and timely clinical documentation in compliance with applicable regulations and payer requirements
Preferred Experience / Skills
Prior experience in an outpatient physical therapy setting
Familiarity with functional quality indicators, letters of medical necessity, and insurance authorization processes
Demonstrated commitment to professional development through continuing education and involvement in professional organizations
