Key Responsibilities
- Evaluate and assess patients to develop individualized physical therapy treatment plans within a skilled nursing facility setting
- Deliver hands-on therapeutic interventions to improve patient mobility, strength, and functional independence
- Monitor and document patient progress, adjusting treatment plans as clinically appropriate
- Collaborate with interdisciplinary care teams to coordinate patient care and discharge planning
- Educate patients and their families on therapeutic exercises and home safety strategies
Required Qualifications
- Active Physical Therapist (PT) license in the state of Washington or eligibility to obtain
- Graduate of an accredited Physical Therapy program
- Experience working in a skilled nursing facility (SNF) setting
- Current CPR/BLS certification
Preferred Experience / Skills
- Prior travel or contract therapy experience
- Familiarity with SNF documentation and Medicare compliance requirements
