Key Responsibilities

• Evaluate and assess patients to develop individualized physical therapy treatment plans within a skilled nursing facility setting
• Deliver hands-on therapeutic interventions to improve patient mobility, strength, and functional independence
• Monitor and document patient progress, adjusting treatment plans as clinically appropriate
• Collaborate with interdisciplinary care teams to coordinate patient care and discharge planning
• Educate patients and their families on therapeutic exercises and home safety strategies

Required Qualifications

• Active Physical Therapist (PT) license in the state of Washington or eligibility to obtain
• Graduate of an accredited Physical Therapy program
• Experience working in a skilled nursing facility (SNF) setting
• Current CPR/BLS certification

Preferred Experience / Skills

• Prior travel or contract therapy experience
• Familiarity with SNF documentation and Medicare compliance requirements
