Key Responsibilities
  • Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans based on patient needs, goals, and physician direction.
  • Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines.
  • Ensure all care is provided under signed, valid physician orders and within insurance approvals as required for clinical care.
  • Design functional treatment programs that support effective patient return to community activities and progress patients toward discharge within an established length of stay.
  • Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations.
  • Adhere to discipline-specific State Practice Act and regulatory requirements, maintaining professional licensure in good standing and completing all state-mandated continuing education requirements.
Required Qualifications
  • Active and unrestricted Physical Therapist license in the state of Illinois.
  • Ability to perform comprehensive patient evaluations and develop evidence-based treatment plans.
  • Proficiency in clinical documentation, goal writing, and compliance with state and federal guidelines.
  • Knowledge of and adherence to discipline-specific professional codes of conduct and ethical standards.
  • Demonstrated commitment to patient safety and safe patient handling practices.
Preferred Experience / Skills
  • Prior experience in an outpatient physical therapy setting.
  • Experience supervising or mentoring physical therapy students.
  • Active involvement in continuing education and professional development activities.
