Key Responsibilities

• Conduct 60-minute initial evaluations and 30-minute follow-up sessions for assigned outpatient caseload
• Develop, implement, and modify individualized treatment plans aligned with patient needs, goals, and physician direction
• Complete thorough medical record documentation including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
• Delegate and direct discipline-specific therapy services for assigned patients, ensuring functional treatment programs support return to community activities
• Educate patients, families, students, and coworkers as part of a comprehensive care approach
• Adhere to the state Physical Therapy Practice Act, maintain licensure in good standing, and fulfill all continuing education requirements

Required Qualifications

• Active Physical Therapist license in good standing in the state of Colorado
• Completion of all state-mandated continuing education requirements
• Ability to perform comprehensive patient evaluations and develop evidence-based treatment plans
• Compliance with applicable professional codes of conduct and ethical standards

Preferred Experience / Skills

• Prior experience in an outpatient therapy setting
• Experience supervising or mentoring PT students in a clinical environment
• Familiarity with performance improvement methodologies and quality initiatives
