    Contract | Full-Time (40 hours/week) | Days | Onsite | Outpatient

    Key Responsibilities
    • Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
    • Conduct 60-minute evaluations and 30-minute follow-up visits; progress patients toward discharge within an established length of stay
    • Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
    • Delegate and direct discipline-specific therapy services for assigned patients; ensure care is provided under signed, valid physician orders and within insurance approvals
    • Serve as an educator to patients, families, students, and coworkers; participate in student programs as assigned, providing supervision, learning objectives, and student evaluations
    • Adhere to discipline-specific State Practice Act and regulatory agency requirements; actively pursue professional development through continuing education and professional activities
    Required Qualifications
    • Active and unrestricted Physical Therapist license in good standing in the state of Colorado
    • Completion of all state-mandated continuing education requirements
    • Demonstrated ability to perform comprehensive patient evaluations and develop evidence-based treatment plans
    • Knowledge of and adherence to discipline-specific professional code of conduct and ethical standards
    • Proficiency in medical record documentation in compliance with state and federal guidelines
    Preferred Experience / Skills
    • Prior experience in an outpatient physical therapy setting
    • Experience supervising or mentoring physical therapy students
    • Familiarity with performance improvement methodologies and quality initiatives
