Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
Conduct 60-minute evaluations and 30-minute follow-up visits; progress patients toward discharge within an established length of stay
Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
Delegate and direct discipline-specific therapy services for assigned patients; ensure care is provided under signed, valid physician orders and within insurance approvals
Serve as an educator to patients, families, students, and coworkers; participate in student programs as assigned, providing supervision, learning objectives, and student evaluations
Adhere to discipline-specific State Practice Act and regulatory agency requirements; actively pursue professional development through continuing education and professional activities
Required Qualifications
Active and unrestricted Physical Therapist license in good standing in the state of Colorado
Completion of all state-mandated continuing education requirements
Demonstrated ability to perform comprehensive patient evaluations and develop evidence-based treatment plans
Knowledge of and adherence to discipline-specific professional code of conduct and ethical standards
Proficiency in medical record documentation in compliance with state and federal guidelines
Preferred Experience / Skills
Prior experience in an outpatient physical therapy setting
Experience supervising or mentoring physical therapy students
Familiarity with performance improvement methodologies and quality initiatives
