    Contract | Full-Time (40 hours/week) | Flexible Schedule | Onsite | Day Shift

    Key Responsibilities
    • Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
    • Conduct 60-minute evaluations and treatment sessions in an outpatient setting, progressing patients toward discharge within an established length of stay
    • Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
    • Delegate and direct discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities
    • Serve as an educator for patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations as assigned
    • Adhere to the discipline-specific State Practice Act and all regulatory requirements; maintain professional licensure in good standing and fulfill all state-mandated continuing education requirements
    Required Qualifications
    • Active and unrestricted Physical Therapist license in the state of California
    • Outpatient Physical Therapy experience required
    • Demonstrated ability to perform comprehensive patient evaluations and develop evidence-based treatment plans
    • Proficiency in medical record documentation in compliance with state, federal, and payer guidelines
    • Commitment to patient and employee safety standards and safe patient handling practices
    Preferred Experience / Skills
    • Experience supervising or mentoring Physical Therapy students in a clinical setting
    • Familiarity with performance improvement methodologies and participation in quality improvement initiatives
    • Active involvement in professional organizations or ongoing continuing education beyond state requirements
