Contract | Full-Time | Days | Onsite | 5 x 8-hour shifts per week, including rotating weekends and holiday rotation

Key Responsibilities

• Perform Start of Care (SOC), Resumption of Care (ROC), recertifications, evaluations, and daily patient visits in a home health setting
• Manage a caseload of approximately 6.5 points per day within the assigned coverage area
• Treat patients with diagnoses including CHF, orthopedic conditions, and pneumonia
• Travel to patient homes within the designated service area to deliver skilled physical therapy services
• Participate in rotating weekend shifts (8:00 AM – 5:00 PM) and holiday rotation as scheduled

Required Qualifications

• Active Physical Therapist (PT) license in the state of Kentucky
• Current BLS certification
• Ability to perform home health visits independently in the field
• Willingness to participate in weekend and holiday rotation

Preferred Experience / Skills

• Prior home health experience preferred
• Experience treating patients with CHF, orthopedic, or pulmonary diagnoses
• Candidates with at least some clinical experience beyond new graduation are preferred
