    Contract | Full-Time | Days | Outpatient | 40 hours per week

    Key Responsibilities
    • Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
    • Conduct 60-minute evaluations and 30-minute follow-up sessions, progressing patients toward discharge within an established length of stay
    • Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
    • Delegate and direct discipline-specific therapy services for assigned patients, ensuring design of functional treatment programs that support effective return to community activities
    • Serve as an educator for patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations
    • Adhere to the applicable State Practice Act and regulatory requirements, maintain professional licensure in good standing, and uphold discipline-specific professional codes of conduct
    Required Qualifications
    • Active and unrestricted Physical Therapist licensure in the state of Colorado
    • Demonstrated ability to perform comprehensive patient evaluations and develop evidence-based treatment plans
    • Proficiency in clinical documentation including goal writing, functional assessments, and compliance with state and federal guidelines
    • Knowledge of and adherence to safe patient handling practices and workplace safety standards
    • Ability to work within physician orders and insurance authorization requirements
    Preferred Experience / Skills
    • Prior experience in an outpatient physical therapy setting
    • Experience supervising or mentoring physical therapy students
    • Active engagement in continuing education and professional development activities
