Contract | Full-Time | Days | Outpatient | 40 hours per week
Key Responsibilities
Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
Conduct 60-minute evaluations and 30-minute follow-up sessions, progressing patients toward discharge within an established length of stay
Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
Delegate and direct discipline-specific therapy services for assigned patients, ensuring design of functional treatment programs that support effective return to community activities
Serve as an educator for patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations
Adhere to the applicable State Practice Act and regulatory requirements, maintain professional licensure in good standing, and uphold discipline-specific professional codes of conduct
Required Qualifications
Active and unrestricted Physical Therapist licensure in the state of Colorado
Demonstrated ability to perform comprehensive patient evaluations and develop evidence-based treatment plans
Proficiency in clinical documentation including goal writing, functional assessments, and compliance with state and federal guidelines
Knowledge of and adherence to safe patient handling practices and workplace safety standards
Ability to work within physician orders and insurance authorization requirements
Preferred Experience / Skills
Prior experience in an outpatient physical therapy setting
Experience supervising or mentoring physical therapy students
Active engagement in continuing education and professional development activities
