Contract | Days | Monday – Friday, 8:00 AM – 5:00 PM | On-Call Required | 13-Week Assignment
Key Responsibilities
- Provide physical therapy services to patients in a home health setting
- Conduct evaluations and develop individualized treatment plans
- Meet productivity expectations of 30 points per week
- Participate in on-call rotation as required
- Complete documentation accurately using HCHB and OASIS platforms
Required Qualifications
- Active Physical Therapist (PT) license in the state of Ohio
- Minimum of 12 months of home health experience within the past 3 years
- Proficiency with HCHB documentation system
- OASIS competency required
- Ability to meet a 40-hour per week commitment
Preferred Experience / Skills
- Experience working in a home health environment beyond the minimum requirement
- Strong time management and organizational skills in an independent work setting
