Contract | 13 Weeks | Full-Time | Onsite | Days | Monday – Friday, 8:00 AM – 5:00 PM | On-Call Required
Key Responsibilities
- Provide physical therapy services to home health patients in accordance with plan of care
- Complete and maintain OASIS assessments as required
- Utilize HCHB (HomeCare HomeBase) electronic medical records system for documentation
- Meet productivity expectations of 30 points per week
- Participate in on-call rotation as required
Required Qualifications
- Active Physical Therapist (PT) license in the state of Ohio
- Minimum of 12 months of home health experience within the past 3 years
- Proficiency with HCHB (HomeCare HomeBase) documentation system
- OASIS competency required
- Ability to meet 40 hours per week commitment
Preferred Experience / Skills
- Experience with productivity-based home health models
- Strong time management and organizational skills for managing a home health caseload
