Key Responsibilities

• Evaluate and treat patients in a Skilled Nursing Facility (SNF) setting
• Develop and implement individualized physical therapy treatment plans
• Monitor and document patient progress toward established therapy goals
• Collaborate with interdisciplinary care teams to support patient outcomes
• Ensure compliance with facility policies and applicable regulatory standards

Required Qualifications

• Active Physical Therapist (PT) license in the state of Wisconsin
• Graduate of an accredited Physical Therapy program
• Current CPR/BLS certification
• Experience working in a clinical or rehabilitation setting

Preferred Experience / Skills

• Prior experience in a Skilled Nursing Facility (SNF) environment
• Familiarity with Medicare documentation and compliance requirements
