Key Responsibilities

• Provide physical therapy services to participants enrolled in a PACE (Program of All-inclusive Care for the Elderly) program
• Conduct comprehensive physical therapy evaluations and develop individualized treatment plans
• Deliver therapeutic interventions aimed at maintaining or improving participants' functional mobility and independence
• Collaborate with an interdisciplinary care team to coordinate participant care and treatment goals
• Document evaluations, progress notes, and treatment plans in accordance with regulatory and program requirements

Required Qualifications

• Current, active Physical Therapist (PT) license in the state of Indiana
• Graduate of an accredited Physical Therapy program
• Experience providing physical therapy services in a clinical or rehabilitative setting
• Ability to work effectively within an interdisciplinary team environment

Preferred Experience / Skills

• Prior experience working with geriatric or elderly patient populations
• Familiarity with PACE program models or all-inclusive care environments
