Contract | Days | Monday–Friday | 36 hours per week minimum

Key Responsibilities

• Provide direct physical therapy services to patients across assigned facility locations
• Evaluate patients and develop individualized treatment plans to address functional deficits
• Deliver therapeutic interventions in accordance with physician orders and plan of care
• Document patient progress, treatment notes, and discharge planning in a timely manner
• Collaborate with interdisciplinary care teams to coordinate patient care
• Maintain compliance with facility policies and applicable regulatory standards

Required Qualifications

• Current and active Physical Therapist (PT) license in the state of Montana
• Graduate of an accredited Physical Therapy program
• Current CPR/BLS certification
• Ability to fulfill a 12-week contract assignment

Preferred Experience / Skills

• Prior experience in a skilled nursing, rehabilitation, or long-term care setting
• Experience working across multiple care sites or split-facility assignments
