Key Responsibilities

• Provide physical therapy services to patients in a home health setting within an assigned territory in North Napa County
• Meet a minimum productivity expectation of 30 points per week
• Complete all clinical documentation by 10:00 AM the following day
• Participate in a weekend rotation as part of the standard schedule

Required Qualifications

• Active Physical Therapist (PT) license in the state of California
• Minimum of 1 year of home health experience
• Proficiency with HCHB (Homecare Homebase) software is required

Preferred Experience / Skills

• Familiarity with home health territory management and patient scheduling
• Strong time management skills with the ability to meet documentation deadlines
