Physical Therapist (PT) – Home Health

Contract | Full-Time | Days | Onsite / Field-Based
Key Responsibilities
- Prioritize and conduct start of care visits with next-day mandatory reporting
- Complete clinical notes and all required documentation within 48 hours of each visit
- Supervise Physical Therapy Assistants in the field
- Maintain timely communication with supervisor, intake, and office staff during normal business hours
- Meet a productivity requirement of 30 points per week, with caseloads and visit mixes averaging 8 hours per day and 40 hours per week
- Utilize Kinnser documentation system for all clinical recordkeeping
Required Qualifications
- Active California Physical Therapist (PT) license
- Minimum of one year of home health experience
- Ability to cover the assigned territory and expand coverage area as needed
Preferred Experience / Skills
- Experience with OASIS documentation including start of care, recertification, resumption of care, and discharge assessments
- Familiarity with the Kinnser electronic documentation platform
- Experience supervising Physical Therapy Assistants in a home health setting
