Key Responsibilities

• Evaluate and treat patients in both skilled nursing and outpatient settings
• Develop and implement individualized physical therapy treatment plans
• Monitor and document patient progress toward established goals
• Collaborate with interdisciplinary care teams to ensure optimal patient outcomes
• Maintain accurate and timely clinical documentation in accordance with facility standards

Required Qualifications

• Active Physical Therapist (PT) license in the state of California
• Experience in a Skilled Nursing Facility (SNF) setting
• Experience in an Outpatient (OP) setting
• Ability to fulfill a 35-hour weekly guarantee

Preferred Experience / Skills

• Prior contract or travel therapy experience
• Strong adaptability to varied patient populations and care environments
