Key Responsibilities

• Provide direct physical therapy services to patients in an outpatient or clinic-based setting
• Evaluate, assess, and develop individualized treatment plans for patients
• Deliver therapeutic interventions in alignment with established care protocols
• Complete required clinical documentation accurately and in a timely manner
• Collaborate with interdisciplinary care teams to support patient progress and outcomes

Required Qualifications

• Active Physical Therapist license required; must obtain permanent Arizona licensure promptly upon assignment start
• Prior experience in a physical therapy clinical setting
• Ability to meet credentialing and contract quality assurance requirements
• Must not have worked at this facility in a permanent or PRN capacity within the past year

Preferred Experience / Skills

• Experience working in a high-volume outpatient or clinic-based physical therapy environment
• Familiarity with travel or contract assignment workflows and documentation requirements
