Key Responsibilities- Evaluate and treat patients in a Skilled Nursing Facility (SNF) setting
- Develop and implement individualized physical therapy treatment plans
- Monitor and document patient progress toward established therapy goals
- Collaborate with interdisciplinary care teams to coordinate patient care
- Educate patients and family members on therapeutic exercises and home programs
Required Qualifications- Active Physical Therapist (PT) license in the state of Colorado
- Graduate of an accredited Physical Therapy program
- Current BLS/CPR certification
- Experience working in a SNF or post-acute care environment
Preferred Experience / Skills- Prior experience in a travel or contract therapy role
- Familiarity with skilled nursing documentation and Medicare guidelines
