Key Responsibilities

• Provide physical therapy services to patients in home environments, assisted living facilities, senior apartments, and board and care homes
• Conduct Start of Care visits, evaluations, and follow-up visits, averaging 3–5 patients per day
• Complete OASIS assessments and maintain thorough documentation within required shift hours (8:30 AM – 5:00 PM)
• Supervise up to two PTAs and/or COTAs as assigned
• Manage a patient caseload spanning varied diagnoses including orthopedic, cardiopulmonary, oncology, neurological, geriatric, and amputation cases (no pediatrics or hospice)
• Participate in a structured 3-week orientation including online learning and field precepting prior to independent practice

Required Qualifications

• Active California Physical Therapist (PT) license
• Current BLS certification
• Active National Provider Identifier (NPI) number
• Minimum 2 years of experience in a healthcare setting
• Home Health experience including OASIS documentation required
• Valid driver's license and current proof of automobile insurance
• Strong oral, written, and organizational communication skills
• Ability to work in varied home environments that may include limited space, temperature extremes, or cluttered conditions

Preferred Experience / Skills

• Community or home health experience strongly preferred
• Experience with HomeCare HomeBase (HCHB) electronic medical records system
