Key Responsibilities

• Evaluate and treat patients in a Skilled Nursing Facility (SNF) setting
• Develop and implement individualized physical therapy treatment plans
• Monitor and document patient progress toward therapy goals
• Collaborate with interdisciplinary care teams to coordinate patient care
• Educate patients and family members on therapeutic exercises and home programs

Required Qualifications

• Active and unrestricted Physical Therapist license in the state of Washington
• Graduate of an accredited Physical Therapy program
• Current BLS/CPR certification
• Experience working in a Skilled Nursing Facility or similar post-acute care setting

Preferred Experience / Skills

• Prior travel or contract therapy experience
• Familiarity with SNF documentation and Medicare guidelines
