Key Responsibilities

• Evaluate and treat patients in accordance with individualized physical therapy plans of care
• Deliver skilled physical therapy services across a 7-day schedule (Sunday through Saturday)
• Document patient progress, treatment plans, and outcomes in compliance with facility and regulatory standards
• Collaborate with interdisciplinary care teams to support patient rehabilitation goals
• Maintain current Physical Therapist licensure and adhere to all applicable state and professional practice standards

Required Qualifications

• Active Physical Therapist (PT) license in the state of California
• Graduate of an accredited Physical Therapy program
• Ability to work a flexible schedule including weekends

Preferred Experience / Skills

• Prior experience in a skilled nursing facility or long-term care setting
• Familiarity with Medicare documentation and compliance requirements
