Physical Therapist – Home Health

Schedule / Job TypeContract | Days (5x8 Hours, 8:00–17:00)
Key Responsibilities- Provide physical therapy services to patients within an assigned home health territory
- Complete patient visits in accordance with the established plan of care
- Perform and document OASIS assessments accurately and in a timely manner
- Maintain timely clinical documentation in compliance with regulatory and organizational standards
- Participate in on-call rotation as required
- Complete required orientation modules during the onboarding period
Required Qualifications- Active California Physical Therapist license (pending CA license considered if application is submitted prior to submission)
- Minimum of 1 year of Home Health experience
- BLS certification through the American Heart Association (AHA)
- Active driver's license and access to personal transportation
- Ability to meet timely communication and documentation requirements
- Completion of Home Health checklist is mandatory
Preferred Experience / Skills- OASIS experience strongly preferred; candidates with solid outpatient experience may be considered
- Outpatient physical therapy experience considered in lieu of home health background
- California license in hand at time of submission preferred
