Key Responsibilities
- Provide physical therapy services to adult patients in a home health setting across the Montgomery and upper Prince George's County areas
- Conduct approximately 6 patient visits per day within the assigned service area
- Evaluate, plan, and implement individualized physical therapy treatment plans in accordance with physician orders and patient needs
- Document patient care accurately and in a timely manner using electronic health record systems
- Maintain compliance with all applicable clinical and regulatory standards throughout the duration of the assignment
Required Qualifications
- Active Maryland Physical Therapist state license or Compact license — must be active and in hand at time of submission
- Current BLS certification — expiration date must be included at submission
- Minimum of 1 year of physical therapy experience
- Home health experience required
- Must be nicotine-free; a positive nicotine screen will result in cancellation
- Two supervisor references required
Preferred Experience / Skills
- Experience with HCHB (HomeCare HomeBase) electronic documentation system preferred
- Familiarity with adult home health case management and high-volume visit scheduling
