Contract | Full-Time | Onsite | Day Shift (Monday–Friday, 8:00 AM–5:00 PM, No Weekends or Holidays)

Key Responsibilities

• Provide physical therapy services to patients in a home health setting across Montgomery County and lower Frederick County
• Complete OASIS paperwork accurately and in a timely manner
• Utilize HCHB (Pointcare) electronic documentation system for patient records and care coordination
• Cover caseload during PTO periods, ensuring continuity of patient care
• Perform evaluations, develop treatment plans, and deliver skilled therapy interventions within scope of practice

Required Qualifications

• Active Maryland state license or Compact PT license — must be active and in hand at time of submission
• Experience with HCHB (Pointcare) electronic documentation system
• Demonstrated experience completing OASIS paperwork
• Two supervisor references required
• Must be nicotine-free (nicotine screening required; positive result will result in cancellation)

Preferred Experience / Skills

• Prior home health physical therapy experience
• Familiarity with WorkDay for completing compliance modules
