Contract (13 Weeks) | Monday – Friday, 8:00 AM – 5:00 PM | Onsite / Field-Based

Key Responsibilities

• Provide skilled physical therapy services to patients in a home health setting across an assigned territory
• Conduct evaluations, develop individualized treatment plans, and deliver therapeutic interventions
• Monitor and document patient progress using the HCHB (HomeCare HomeBase) EMR system
• Collaborate with interdisciplinary care teams to coordinate patient care and ensure positive outcomes
• Manage a caseload across Spokane County and potentially lower Stevens County

Required Qualifications

• Active and unrestricted Physical Therapist (PT) license in the state of Washington
• Minimum of 12 months of Home Health experience within the past 3 years
• Proficiency or ability to work within the HCHB EMR platform
• Valid driver's license and reliable transportation for field-based patient visits

Preferred Experience / Skills

• Familiarity with Spokane County geography and surrounding areas
• Strong time management and organizational skills for independent caseload management
• Experience working autonomously in a community or field-based therapy setting
