Physical Therapist – Home Health (Travel Contract)

Stockton, CA (Central Valley – includes surrounding areas such as Modesto)

Schedule / Job Type

Contract | Days | Full-Time | Onsite / Field-Based | 5x8-Hour Shifts (8:00 AM – 5:00 PM) | Schedule options include Tuesday–Saturday, Wednesday–Sunday, or Monday–Friday

Key Responsibilities

• Provide physical therapy services to patients in a home health setting across an assigned geographic territory
• Complete OASIS assessments accurately and in a timely manner
• Maintain timely clinical documentation in accordance with regulatory and organizational standards
• Manage an independent caseload with travel between patient homes within the assigned coverage area
• Participate in on-call rotation as required
• Complete required orientation modules, which are billable and conducted during orientation

Required Qualifications

• Active California Physical Therapist license (candidates with a pending CA license who have applied at time of submission will be considered)
• BLS certification through the American Heart Association (AHA) required
• OASIS experience required
• Active, valid driver's license and personal reliable transportation required
• Minimum of 1 year of home health experience
• Timely communication and documentation are mandatory

Preferred Experience / Skills

• California license in hand at time of submission preferred
• Candidates without home health experience will be considered on a case-by-case basis
