Key Responsibilities

• Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction.
• Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines.
• Delegate and direct all discipline-specific therapy services for assigned patients, ensuring design of functional treatment programs that support effective return to community activities.
• Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations.
• Adhere to discipline-specific State Practice Act and all applicable regulatory requirements, maintaining professional licensure in good standing and completing all state-mandated continuing education requirements.
• Support organizational performance improvement initiatives by identifying opportunities for quality enhancement and contributing feedback through appropriate channels.

Required Qualifications

• Active and unrestricted Physical Therapist license in the state of Florida.
• Experience as a licensed Physical Therapist; new graduates will not be considered.
• Proficiency in clinical reasoning, treatment planning, and goal writing within an outpatient setting.
• Knowledge of and adherence to discipline-specific professional codes of conduct and state practice acts.
• Ability to ensure care is provided under signed, valid physician orders and within applicable insurance approvals.

Preferred Experience / Skills

• Prior experience in an outpatient physical therapy setting with a focus on functional rehabilitation and community reintegration.
• Demonstrated commitment to ongoing professional development through continuing education and involvement in professional organizations.
