Key Responsibilities

• Conduct thorough evaluations of patients' physical abilities and limitations.
• Develop and implement personalized treatment plans based on evaluation findings.
• Utilize a variety of therapeutic techniques and modalities to facilitate patient recovery.
• Collaborate with interdisciplinary teams, including physicians, nurses, and occupational therapists, to optimize patient care.
• Educate patients and their families on rehabilitation goals, techniques, and home exercise programs.
• Monitor patient progress and adjust treatment plans as necessary, while maintaining accurate and timely documentation.

Required Qualifications

• Valid and unrestricted Physical Therapist license.
• Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program.
• Confirmed post-graduate experience in a Skilled Nursing Facility (SNF) setting.
• Proficiency in PCC and/or NetHealth documentation platforms.
• Strong interpersonal and communication skills with the ability to work effectively in a team-oriented environment.

Preferred Experience / Skills

• Experience working in a high-volume, fast-paced skilled or long-term care environment.
• Familiarity with current industry best practices and regulatory requirements in skilled/LTC settings.
