Key Responsibilities

• Provide direct physical therapy services to patients in an outpatient therapy setting
• Evaluate, assess, and develop individualized treatment plans based on patient needs and goals
• Implement evidence-based therapeutic interventions to improve patient mobility, strength, and function
• Complete accurate and timely clinical documentation in accordance with facility and regulatory requirements
• Collaborate with interdisciplinary care teams to coordinate patient care and progress

Required Qualifications

• Active Physical Therapist license in the state of Indiana (permanent license required)
• Graduate of an accredited Physical Therapy program
• Prior experience in an outpatient physical therapy setting
• Ability to meet credentialing and compliance requirements prior to assignment start

Preferred Experience / Skills

• Experience working in a high-volume outpatient therapy environment
• Strong documentation skills with familiarity in electronic medical records systems
