Key Responsibilities
Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
Conduct 60-minute initial evaluations and 30-minute follow-up visits in an outpatient setting
Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
Delegate and direct discipline-specific therapy services for assigned patients, ensuring design of functional treatment programs that support return to community activities
Serve as an educator for patients, families, students, and coworkers; participate in student programs as assigned with appropriate supervision and completion of student evaluations
Adhere to discipline-specific State Practice Act and all regulatory requirements; actively pursue professional development through continuing education and professional activities
Required Qualifications
Active and unrestricted Physical Therapist license in good standing in the state of Colorado
Experience performing comprehensive patient evaluations and developing individualized treatment plans
Knowledge of and adherence to discipline-specific professional code of conduct and state practice act requirements
Ability to ensure care is provided under signed, valid physician orders and within insurance approvals
Preferred Experience / Skills
Prior experience in an outpatient physical therapy setting
Experience supervising or mentoring physical therapy students
Familiarity with performance improvement methodologies and quality of care initiatives
