Key Responsibilities

• Manage an average of 2–5 patient visits per day, including evaluations, start-of-care assessments, and subsequent visits
• Meet a weekly productivity expectation of 25 points per week
• Document all patient care using the HCHB Pointcare electronic medical records system
• Provide supervision for 1–2 Physical Therapist Assistants (PTAs)
• Travel to patient homes within the East Portland coverage area

Required Qualifications

• Active Physical Therapist (PT) license in the state of Oregon
• Experience in a home health setting
• Ability to travel throughout the East Portland service area
• Ability to complete orientation requirements, including compliance training and ride-alongs

Preferred Experience / Skills

• Prior experience with HCHB Pointcare EMR system
• Experience supervising PTAs in a home health environment
