Physical Therapist (PT) – Home Health

Key Responsibilities- Provide in-home physical therapy services to a caseload of 4–6 patients per day across a defined service area
- Perform patient admissions, ongoing treatment of existing patients, and recertification evaluations
- Treat a multidisciplinary adult patient population including elderly, post-CVA, post-surgical, orthopedic, and wound care cases
- Participate in multidisciplinary team meetings held on Mondays and Wednesdays to discuss current admissions
- Document patient care using Epic EMR with a remote client offshoot
- Begin shift directly from home; travel throughout the assigned service area (up to approximately 60 miles east and west, 60 miles north, and 40 miles south)
Required Qualifications- Active Physical Therapist (PT) license in good standing
- Minimum of 1 year of post-graduate PT experience
- Prior home health experience required
- Orthopedic experience required
- Basic Life Support (BLS) certification through the American Heart Association (AHA)
Preferred Experience / Skills- Post-graduate home health experience strongly preferred
- Experience with Medicare patient populations and rehab-focused care settings
- Familiarity with Epic EMR
