Key Responsibilities
  • Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in collaboration with the interdisciplinary team and in accordance with physician direction.
  • Manage individual, concurrent, and group treatment sessions effectively, ensuring the plan of care is communicated, progressed, and focused on discharge goals within an established length of stay.
  • Complete all medical record documentation in a clear, concise, accurate, and timely manner, including daily notes, goal writing, functional quality indicator assessments, letters of medical necessity, discharge documentation, and charge entry in compliance with state, federal, and intermediary guidelines.
  • Coordinate discharge planning by facilitating equipment trialing and ordering, completing patient and family training, issuing home exercise programs, and arranging for continuing services.
  • Participate in student clinical education programs as assigned, providing supervision, developing learning objectives, and completing student evaluations.
  • Adhere to discipline-specific state practice acts and regulatory requirements, maintaining professional licensure in good standing and fulfilling all continuing education requirements.
Required Qualifications
  • Active and unrestricted Physical Therapist licensure in the state of Ohio.
  • Demonstrated ability to perform comprehensive patient evaluations and develop evidence-based treatment plans.
  • Knowledge of and compliance with state, federal, and payer-specific documentation and billing requirements.
  • Commitment to patient and employee safety standards, including safe patient handling and infection control practices.
  • Ability to collaborate effectively within an interdisciplinary team environment.
Preferred Experience / Skills
  • Experience in an inpatient rehabilitation or acute care hospital setting.
  • Prior experience supervising or mentoring Physical Therapy students.
  • Active involvement in professional organizations or continuing education beyond state-mandated requirements.
