Key Responsibilities

• Provide direct physical therapy services to patients in an outpatient setting
• Evaluate, plan, and implement individualized treatment programs based on patient needs
• Maintain accurate and timely clinical documentation in accordance with facility and regulatory requirements
• Collaborate with interdisciplinary team members to support patient progress and outcomes
• Adhere to all credentialing, compliance, and facility-specific onboarding requirements prior to assignment start

Required Qualifications

• Active Physical Therapist license required; compact license accepted at start but permanent Wisconsin licensure must be obtained promptly
• Experience in an outpatient physical therapy setting
• Must meet all contract credentialing and quality assurance requirements prior to start
• Candidates who have worked permanently or PRN at this facility within the last year are not eligible for this assignment

Preferred Experience / Skills

• Prior travel therapy experience in an outpatient setting
• Strong documentation skills and ability to adapt to facility-specific systems
