Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
Conduct 60-minute evaluations and 30-minute follow-up appointments in an outpatient setting
Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
Design functional treatment programs focused on progressing patients toward discharge and effective return to community activities
Serve as an educator for patients, families, students, and coworkers; participate in student programs as assigned with appropriate supervision and development of learning objectives
Adhere to discipline-specific State Practice Act and all applicable regulatory agency requirements while maintaining professional licensure in good standing
Required Qualifications
Active and unrestricted Physical Therapist license in the state of Colorado
Ability to perform comprehensive patient evaluations and develop evidence-based treatment plans
Proficiency in medical record documentation in compliance with state, federal, and payer guidelines
Knowledge of and adherence to discipline-specific professional code of conduct and State Practice Act
Completion of all state-mandated continuing education requirements
Preferred Experience / Skills
Prior experience in an outpatient physical therapy setting
Experience supervising or mentoring physical therapy students
Familiarity with performance improvement methodologies and quality initiatives
