Key Responsibilities

• Evaluate patients and develop individualized physical therapy treatment plans within a skilled nursing facility setting
• Administer therapeutic interventions to improve patient mobility, strength, and functional independence
• Monitor and document patient progress, adjusting treatment plans as clinically appropriate
• Collaborate with interdisciplinary care teams to coordinate patient care and discharge planning
• Educate patients and family members on home exercise programs and safe functional techniques

Required Qualifications

• Active Physical Therapist (PT) license in the state of Minnesota or eligibility to obtain licensure
• Graduate of an accredited Physical Therapy program
• Experience working in a skilled nursing facility (SNF) or long-term care setting
• Strong clinical documentation skills and knowledge of regulatory compliance standards

Preferred Experience / Skills

• Prior contract or travel therapy experience
• Familiarity with MDS documentation and PDPM reimbursement models
